Regular
Form - For New Students or
Former Students who have not completed
any courses for three or more years.
Re-Admission
Form - Former Students who
have been away from studies for one or
more semesters.
Re-Enrollment
Form - Former students who
have only been away from Master’s
for either the Fall or Winter semester
can complete the Re-Enrollment Form,
rather than the Re-Admission Application
Form.
Occasional
Student Form - For students who want to do 1-5
courses without having to complete the
Regular Application Form.

Completion Program Form - All students
applying to the Bachelor of Theology Completion
program must complete this application.
