Summer 2008 Course Registration Form

Students wishing to be full-time must register for at least 12 credit hours. This form may be used to register for courses taught on campus or online.

Summer 2008 Registration Form

Part A: General Information (All Students must complete this section)

1. If you were a student in Winter 2008 please proceed to number 2. If not one of the following will apply to you:

New Student, please ensure you have completed the Regular Application Form or you have completed the online Occasional Student Application Form.
Former Student, please complete an Re-Admission Application Form or a Re-Enrollment Form.
Occasional Student who has completed or will complete five courses prior to the start of the Winter semester will need to complete a Regular Application Form.

2. Mature Adults Only: Are you 55 years old or older? Yes No If yes, please enter your date of birth:(DD/MM/YYYY) . You are eligible for a Mature Adult discount (See the Summer '08 Registration Procedures for more information). If you have not previously done so, please fax(416-482-7004) or mail a copy of a government-issued photo identification card so that your tuition reduction can be processed.

3. Payment Options: Indicate which of the following Payment Options you will be using to pay for your courses. Additional details regarding the Payment Options was explained on the Summer '08 Registration Procedures page:

On or before April 10, 2008 I will arrange with the Finance Office to pay for my fees in full by cash, cheque, debit card or credit card.
On or before April 10, 2008 I will arrange with the Finance Office to use Deferred Payment Plan.
On or before April 10, 2008 I will arrange interim financing by contacting , since I have not received my government-issued Student Loan.
